I would like to thank those who contributed to my early thinking about social
work, especially Edith Close, my first social work supervisor and Alfred Kahn,
Professor of Social Work at Columbia University. Norman Ginsburg and Georgie
Parry Crooke, my PhD dissertation supervisors, provided invaluable support in
shaping my policy and research perspectives. Stephen Fox and John Gabriel were
sympathetic in negotiating the demands of my social work education role with my
writing efforts. In addition, I would like to thank Sue Lawrence, Karen Lyons, Sam
Whimster, Jean Thomas and the members of the Writing Group at the Department
of Applied Social Sciences at London Metropolitan University for their discussions
about care management and comments on drafts. I would also like to thank students
